Tahrima Khanom, M.S.W.
As the Social Drivers of Health Manager, Tahrima leads MSHIELD’s community-clinical linkage work and facilitates the development, implementation, and evaluation of social needs screening-linkage interventions. Tahrima is also responsible for maintaining key partnerships and providing consulting and technical assistance to the CQI portfolio. Tahrima has extensive experience developing and leading public health programs and projects to improve health outcomes for LGBTQ+, immigrant, and adolescent populations. She has worked in a variety of settings including federally qualified health centers, community-based organizations, and health departments. Prior to joining the team at MSHIELD, Tahrima served as the Program Manager for the Adolescent Health Initiative, a nonprofit organization committed to transforming the health care landscape for young people. Tahrima received her MSW from the University of Michigan and her BA in Psychology from Wayne State University. With a career that spans both direct care work and management, Tahrima is passionate about data-driven, community-led solutions to society’s most pertinent problems. In her spare time, Tahrima enjoys knitting, traveling, and spending time with her husband and their 1-year-old daughter.
